2. Letter from Isidro P. Lorenzo, Jr., Manager of Olongapo Subic Castillejos San Marcelino Transport Service and Multi-Purpose Cooperative, Inc., furnishing a copy of the Annual Report of OSCSMTSMPCI including Program/Order of Business in connection with the forthcoming Annual General Assembly on April 17, 2004.
3. Letter from Evelyn DJ. Delos Santos, Officer-In-Charge, Olongapo City Labor And Employment Center requesting that the SPES Mariel Anne O. Tiongson be pulled out for transfer to the UBSP-REACH office.

7. Copy of the letter from the Purok Leaders Association of Barangay East Bajac-Bajac, Olongapo City requesting for the increase in the Purok Leader’s honorarium from PhP500.00 to PhP1,000.00 and the provision for their uniforms.
8. Annual Budget of New Kalalake for the Calendar Year 2004, which was approved on 15th day of March 2004 by the Sangguniang Barangay of New Kalalake under Resolution No. 008, Series of 2004.
9. First Indorsement of the following:
a. Letter of suspended Barangay Captain Danilo S. Fernandez dated April 15, 2004 requesting that the six (6) month preventive suspension meted on him, as per Sangguniang Panlungsod Resolution No. 195, be lifted if not lessen to a period of three (3) months;
b. Letter of Gerardo A. David, Supervisor, F.O. Smart Communications (Bataan-Zambales Area) dated April 14, 2004 requesting a resolution from the City Council to allow the installation of additional cell sites in Barangays Gordon Heights, New Cabalan, Kalaklan, East Tapinac and Pag-asa to address the weak indoor signal being experienced by subscribers, as part of SMART’s expansion scheme in the city;
c. Letter of Len Rutledge, Team Leader, Sinclair Knight Merz (Philippines), Inc., Ortigas Centre, Pasig City, dated April 16, 2004 in response to the letter of the City Mayor dated April 1, 2004 clarifying issues concerning the modernization of the city’s Public Utilities Department (PUD) through privatization; and

This pertains to the requirement for additional appropriation for the purchase of air conditioner units for the newly constructed semi-private room annex and various hospital equipment at the James L. Gordon Memorial Hospital.
